We are looking for 2 individuals to work in our part-time lines at our group home in Ashwood. We have 5 residents within the house. You will be responsible for providing support with activities of daily living and developing, implementing, and monitoring Individual Plans including personal care and implementation of complex care plan requirements as directed by health professionals. Clear and strong written communication skills are required to ensure reports and client notes are as detailed as possible. In these roles you will work to facilitate the delivery of quality services and achieve positive outcomes with and for the clients within the house.
