About the role

This is a challenging but rewarding opportunity for you to make a positive change in the lives of People with a Disability.

Working in a team, you will facilitate the delivery of quality services and achieve positive outcomes with service users in a positive and proactive environment. You will be actively supporting their individual needs and creating a safe environment. You will be responsible for supporting and promoting their skills and abilities, implementing and monitoring their Support Plan. 

This role will services the Ipswich Region. You will be required to travel to multiple locations.

Please note: In order to best support our clients, the successful incumbent for this position will be required to do sleepover shifts, work weekends and must be flexible with working arrangements.

Key Responsibilities

  • Day to day care (including personal care) and supervision
  • Community Access & Transport
  • Maintain & Monitor Person Centred Plans
  • Documenting & Progress Note keeping

Skills & Experience

  • Unrestricted Australian Drivers Licence - Essential
  • Current Provide First Aid Certificate (or willingness to obtain) - Essential
  • Certificate III in Individual Support / Aged Care / Community Services or higher
  • Passionate about delivering Person-Centred care

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
