About the team

AccessCare's Home Support team delivers in-home support to persons living across the Kingston and Frankston LGA's. The team includes Support Workers, Client Services Officers /Rosterers and Team Support Officers working together to oversee coordination and delivery of a range of programs and services that support our clients in their preference to remain living at home.

About the role

The Home Support Team Coordinator is responsible for staff leadership and the delivery of services which are responsive to consumer need, reflect best practice and demonstrate the highest level of customer service. You will be skilled in the provision of client-driven care, balanced with an advanced understanding of Health and Safety awareness, and the challenges faced by Support Workers working in the community. Experience working with rostering in community care is also desirable.

Other Responsibilities include:

  • Staff Management: Selection and recruitment of suitably skilled Support Workers is a pivotal aspect of this role. This includes all aspects of recruitment from the point of advertising, through AccessCare's induction processes, planning and coordinating training and providing on-going monitoring and support.
  • Client Contact and Service Delivery: Current understanding of the service delivery sector is essential. The Coordinator is responsible for ensuring optimal care that supports client goals and family/carer needs and promotes wellness and re-ablement. This includes ensuring there is adequate daily cover across AccessCare's community services including Commonwealth Home Support, Support at Home, and HACCPYP.
  • Supporting on-going change: In an environment where change is a constant factor, you will need to work with the broader AccessCare team to support all staff, both office and community based, to negotiate and adjust to change as the funding and service delivery models continue to evolve.
  • Managing compliance: Ensure that members of the team comply with policies and procedures, both internal and external. This includes reporting requirements, achievement of targets, work health safety requirements and AccessCare's work practices.

Your Skills and Experience

  • A tertiary qualification in Business Management, Social Sciences, Health of a related discipline, or extensive, recent experience in a similar role
  • Demonstrated experience providing training, guidance and leadership to a team to ensure best practice service delivery in community care
  • Understanding of the challenges faced by Support Workers in the field and by rostering staff
  • An ability to effectively manage staff and client concerns, to ensure continuity of care and worker safety
