Kyabra operates an integrated model and works with individuals, families and carers with sometimes complex issues. This work may include support and advocacy through the systems which significantly impact on individual and family lives, such as; child protection; income support; housing; mental health and healthcare; disability; education and legal.
The Intake and Assessment team is responsible for providing a single point of entry for information, assessment and referral to Kyabra services and external services.
Alongside the Manager, the Coordinator, Intake and Assessment will support the day-to-day team operations including program coordination and support and supervision of staff. This role is responsible for the intake and assessment of the Foster & Kinship Care and NDIS portfolios which includes recruitment and initial assessment of foster and kinship carers, out of home placements and initial assessment, and on-boarding of NDIS participants. This role will be required to work closely with other Kyabra Coordinators, liaise with stakeholders and network meetings.
The position is also required to participate in the organisation’s after-hours on call roster. Additional remuneration applies. It is also a requirement to participate in a roster of working from Kyabra’s Richlands office.
A Bachelor degree in Social Work or similar is an essential requirement for this position.
Salary and conditions are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, Level 5 ($47.50 - $49.65 per hour). We also offer:
