Services include Disability, Mental Health, Youth and Family Services, Aged Residential Care, Assessment Referral and Individual Support Services, Community Aged Care and Holocaust Survivors Programs.

About the Role

Jewish Care’s Disability Services provide an individualised, active-support service model of community-based support to people living with a disability.

We are looking for a motivated and well organised individual to provide preplanning, advice and Support Coordination for people entering and sustaining services under the National Disability Insurance Scheme. You will manage an assigned case load to support our clients to choose and control the supports they require. You will have the ability to empower our clients’ participation in all areas of their lives, including personal and decision-making, dignity and respect, relationship building and skill development to promote full community inclusion.

You will be working within a small team of dedicated and passionate individuals in a fast paced and ever changing environment. The role has an outreach focus therefore is a combination of office, community and work from home.

Responsibilities will include:

  • Maintaining effective communication between the client, their funding Planner, and other service providers;
  • Assisting clients to access and utilise all Government and community services they require;
  • Assisting with the preparation of reports to relevant Government Departments;
  • Assisting clients with allocation of their funds to ensure their budget will satisfy their support needs;
  • Maintaining a supportive environment for people according to the principles of inclusion and person centred active support;
  • Participation in team reviews
  • Supporting and empowering vulnerable people to report and speak out against instances of abuse or potential abuse;
  • Undertaking regular monitoring and maintenance of service provision data including client, program and organisational records in accordance with all Jewish Care policies, funding agreements, legal and statutory requirements;
  • Managing compliance and billing in line with NDIS requirements.

Skills and Experience

  • Ideally, you will have experience as a Support Coordinator or experience working within the NDIS environment
  • Relevant qualification or equivalent experience in the disability field or a related area of expertise
  • Demonstrated commitment to disability and human rights
  • Demonstrated experience in building supportive and inclusive environments for people with disability
  • Ability to engage with a diverse range of people, including people with disability, service providers and government departments
  • Ability to effectively manage work schedule, and to work both independently and as part of a team
  • Proficient in the use of Microsoft Office applications
  • Well-developed communication and interpersonal skills
