Jewish Care Victoria, a not-for-profit organisation, is the largest provider of aged and community services for the Jewish Community in Victoria. Services include Disability, Mental Health, Youth and Family Services, Aged Residential Care, Assessment Referral and Individual Support Services, Community Aged Care and Holocaust Survivors Programs.
Jewish Care’s Disability Services provide an individualised, active-support service model of community-based support to people living with a disability.
We are looking for a motivated and well organised individual to provide preplanning, advice and Support Coordination for people entering and sustaining services under the National Disability Insurance Scheme. You will manage an assigned case load to support our clients to choose and control the supports they require. You will have the ability to empower our clients’ participation in all areas of their lives, including personal and decision-making, dignity and respect, relationship building and skill development to promote full community inclusion.
You will be working within a small team of dedicated and passionate individuals in a fast paced and ever changing environment. The role has an outreach focus therefore is a combination of office, community and work from home.
