An exciting opportunity exists for an enthusiastic, innovative worker to provide support coordination for participants of the NDIS team. In this role you will be responsible for assisting participants to utilise to their NDIS plan and help them achieve their short/long term goals. This support role is integral in helping promote autonomy and support in the individual's recovery journey.   

To be suitable for the role, you must possess the following: 

• A relevant tertiary qualification
• Knowledge of the NDIS sector
• Experience and demonstrated ability in the provision of recovery focused rehabilitation to persons with psychosocial disability because of serious mental illness
• To be able to think and reflect critically, continually learn and develop, and adapt to change to provide the best outcomes for the client within their NDIS plan.   
• Demonstrated ability to build strong external partnerships with other providers and help establish and implement support coordination at Star Health
