We are looking for a committed and experienced Antenatal Social Worker to work as part of our innovative Child, Youth and Family Team.

This team provides a range of services to children, young people, families and individuals who may be isolated and disengaged from services. This is a unique role which responds to vulnerable pregnant people with complex health needs in the City of Port Phillip.

The Antenatal Social Worker draws on their discipline-specific knowledge and skills to:

  • Provide assertive outreach to people who are pregnant and who may not otherwise access mainstream health and welfare services.
  • People who access the service may be homeless (or at risk of) and are often dealing with a range of psycho-social issues which can include substance use, family violence, mental illness and financial hardship. People who access the service live, use services or are connected with the City of Port Phillip area.
  • Advocate to other services in relation to the needs of pregnant people and collaborate with internal and external stakeholders to provide high quality, holistic support.
  • Link clients into relevant services including antenatal care, maternal and child health, housing services, family support and other relevant community services.
  • Facilitate group programs or community events for pregnant people or new parents to provide education, build community connection and foster parent-child relationships.

Ideally, you will possess:

  • A professional qualification in Social Work (or equivalent).
  • Experience in the provision of assertive outreach programs and services, particularly with clients with multiple and complex needs.
  • Demonstrated ability to work as a member of a multi-disciplinary team and to work independently as a sole outreach worker.
  • Excellent knowledge of the antenatal care system.
  • A clear understanding of the social model of health.
