The Support Coordinator primary role is to support participants to understand and implement the funded supports in their NDIS plan and to provide linkages with the community, mainstream and other government services. This role plays a pivotal part and focuses on supporting participants to build skills, direct their life as well as connect with providers.

This role requires someone who can:

  • Support an assigned caseloads of participants to implement their NDIS plan, including providing assistance to identify and select providers
  • Monitor plan outcomes and expenditure to ensure the plan remains within budget
  • Strengthen and enhance participant’s ability to self-direct their support and participate in the community
  • Provide timely and accurate reports to the NDIA in participant progress and outcomes
  • Develop and maintain relationships with external stakeholders including NDIA, Local Area Coordinators, service providers
  • Maintain up to date knowledge of the National Disability Insurance Scheme (NDIS) practices and systems
  • Participate in regular case reviews and escalate concerns to NDIS Lead Specialist

Essential criteria

  • Working knowledge of NDIS
  • Experience working with people with disabilities
  • Demonstrated experience using Microsoft Office at an intermediate level
  • Alignment to Intelife Vision, Mission and Values
  • Possess a satisfactory Police Clearance
  • Complete the NDIS Worker Screening Check  
  • Working with children check
  • Previous experience in a Support Coordinator position is a must! – min 6 months experience required

Desirable criteria

  • Relevant Certificate or Diploma qualification 

We ask that you:

  • Have a current WA driver’s licence
  • Be willing to undergo further background checks as required by Intelife
  • Be prepared to participate in pre-employment medical questionnaire  
  • Must have working with children check
