ABOUT THE ROLE

Join Hope Street in Melton as a Case Manager in a program based on the Youth Foyer Like model, providing supported transitional housing for young people and young families. This role offers safe, secure, and affordable accommodation for 18 months, empowering clients to build independence and achieve their goals.

This is a permanent part-time position working a regular work pattern, 60.8 hours per fortnight (0.8 FTE) based in Melton.

In this role you will:

  • provide support within a model of eight one-bedroom units and two-family units.
  • proactively assist young people to develop their skills and abilities to live interdependently in their local community including enjoying meaningful and sustainable relationships.
  • provide strength-based case management support to develop interdependent, sustainable life to access and/or maintain employment, education and training opportunities and achieve meaningful citizen participation.
  • build relationships with key support services to enhance young people’s connections to those services; and
  • maintain accurate client records compliant with DFFH requirements.

ABOUT YOU

You will have:

  • A bachelor’s degree in Social Work, Youth Work, Social Sciences or a relevant health/ welfare discipline.
  • At least 2 years’ experience as a Case Manager, ideally working with youth, and a strong understanding of the youth homelessness service system and allied support services, best placed to support and divert young people from homelessness.
  • Excellent relationship-building skills to motivate young people in transitional settings and help them achieve meaningful goals.
  • A friendly and cooperative approach that enables you to build strong connections with community stakeholders.
  • Well-developed communication skills, including report writing, active listening, negotiation, problem solving, decision making, and advocacy.
  • High proficiency in Microsoft Office and Case Management Software
