Oonah Aboriginal Health and Community Services exists to establish a welcoming and culturally affirming place in Healesville that provides a central point of contact for Community members, for information, services and programs that are focused on building a healthy, strong and skilled Community.

Primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements.

This role is well suited to a person with the following skills and attributes:

  • Good communication and interpersonal skills, with the ability to build trust relationships in community;
  • Experience working with older people;
  • Understanding of the internal ACCHO services available to clients;
  • Demonstrated understanding of the principles of person-centred care and trauma-informed practice;
  • Qualifications in Community Services;
  • Certificate in Aboriginal mental health first aid; and
  • Overarching function of the Aged Care Community Connector is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to Oonah.

This position will report to the Aged Care Access Coordinator. This position is full-time contract role for 12 months and may be extended depending on funding.
