As a result of continuous growth, we have an exciting new opportunity for a Senior Relationship Manager to join our team and support the expansion of our services to Melbourne’s South-Eastern Suburbs.

This role will initially be based from our head office in Malvern East with a view to opening a new office based in the South-East at a later stage. About the role

The Relationship Manager role is core to what makes Hearth different to other support providers.

Relationship Managers are the face of Hearth, supporting individuals with a disability, their family, and support team on a daily basis.

Responsibilities

  • Fostering and continuously developing relationships between participants, their family, and support teams within a defined area
  • Maintaining relationships with support coordinators, house supervisors, and other relevant community and external stakeholders
  • Working collaboratively with our Participants, Support Workers, Support Coordinators and Allied Health professionals to bring NDIS plan goals to life
  • Establish and develop collaborative partnerships with relevant community organisations
  • Lead and support the Support Worker teams to provide responsive, person centred care that enables participants to progress toward their individual care plan goals
  • Accurate case noting and client reviews that reflect forward planning in line with the participant’s NDIS plan
  • Facilitating the support worker Group Interviews and successfully introducing new support workers to participants as required
  • Compliance with Hearth policies and procedures to ensure that a safe working environment is observed
  • Accurate, detailed, and timely reporting and escalation of incidents

Requirements/Selection Criteria:

  • Previous experience in the disability or community services field (Essential)
  • Previous experience working with people from diverse backgrounds (Highly Valued)
  • Qualifications in disability, community services, or related disciplines
  • Demonstrated experience in team leadership and management
  • Strong interpersonal skills and high levels of resilience
  • Ability to work both independently and as part of a team
  • Strong written and verbal communication
  • Working knowledge of Microsoft office suite and employee rostering systems
  • Two contactable professional referees
  • Willing to obtain a Working with Children Check and National Police check (cost covered by Hearth)
