We are currently seeking a Youth Homelessness Case Manager to join our Family & Youth Services team in the Brimbank/Melton region. This is an initial 3 month fixed-term, 4 day per week opportunity with the potential for extension.

About the Role

As a Youth Homelessness Case Manager, you will provide holistic case management, outreach, and housing support to young people aged 15–25 who are experiencing or at risk of homelessness.

Many of these young people present with complex needs and may include young parents and their children. You will work collaboratively with homelessness access points, local networks, and other service providers to deliver client-centred, trauma-informed support that enhances the wellbeing and stability of young people.

Key Responsibilities

  • Provide case management and interim response to young people aged 16–24.
  • Advocate and liaise with housing access points and specialist services across the West and North regions.
  • Deliver services through a strengths-based, client-centered lens.Build and maintain collaborative relationships with local networks and agencies.
  • Maintain accurate records and reporting in line with program requirements.

About You

  • You are a compassionate and proactive professional who thrives in a dynamic, community-focused environment.

You bring:

  • A tertiary qualification in Youth Work, Social Work, or a related field.
  • Experience supporting young people experiencing or at risk of homelessness.
  • Strong understanding of the homelessness service system and youth-specific challenges.
  • Excellent communication, problem-solving, and advocacy skills.
  • A collaborative and flexible approach to working with diverse stakeholders.
