Covering the Central Grampians and Wimmera area, this position will provide an efficient, effective and quality counselling service for the communities across the Central Grampians and Wimmera region. The role necessitates knowledge and use of a range of evidence based holistic approaches to support clients to achieve optimal psychological and physical wellbeing.

This position will support people seeking Early Intervention Chronic Disease Management (EICDM). Intervention is provided within 18 months of diagnosis to people with chronic disease. Support focuses on self-management, health and psychosocial wellbeing. Chronic disease includes diabetes, stroke, asthma, heart and neurological disease.

EICDM support will provide care planning and counselling (and where appropriate shared care planning) paralleled with necessary medical interventions and lifestyle coordination to provide a holistic approach for clients with a chronic disease. Non-clinical interventions often improve mental health, enabling people to cope better with the more difficult aspects of their illness, and therefore are important components of self-management of chronic illnesses to manage symptoms, improve quality of life and slow disease progression.

This positions also provides community health counselling that covers a variety of issues including: grief and loss, depression, anxiety, self-esteem, relationship concerns, parenting and others. Counselling offers personal support strategies for change, someone to trust with difficult concerns and offers hope for a more positive future.
