We are currently seeking a dynamic Program Facilitator to develop and implement a program aimed to enhance the management of heart failure within the community by building GP capacity. Working with a broad range of stakeholders, this position will leverage current resources, information and best practices available to promote patient self management support in relation to heart failure.
Initially working as part of our System Design team you will be responsible for the development of a detailed project, engagement and communications plan which will clearly identify deliverables and targets for the program. Once finalized, you will work within our Program Delivery team to implement the program across the Victorian PHN network.
This is a unique opportunity to make this role your own as we strive to improve patient support for heart failure within the community.
To be successful in this role you will need to have end to end project management experience, from designing and scoping the program to implementation, closing off and evaluation. In addition you will have:
