At Central and Eastern Sydney Primary Health Network, we focus on improving the health and wellbeing of the people who live and work in Sydney, from East to South and the Inner West. Primary health refers to the healthcare provided in the community, such as a local doctor, pharmacist or psychologist. We provide support and training to these health professionals. We help them to connect community members to other services so that everyone involved in a person’s care can know how best to look after their health.

PHNs are not-for-profit organisations. We receive money from the government to provide health services that are right for the people in this area. We also work with hospitals, specialists, aged care facilities and others to improve people’s experience when needing healthcare. We work to make it easier for people to get the best care and support when and where they need it to achieve our vision of healthy and thriving communities.

About this job

  • Work from Office initially with hybrid working arrangements - 2 days required in office

The Aged Care Project Officer will be responsible for commissioning and implementing projects and initiatives that address identified needs and gaps in the CESPHN Older Persons Needs Assessment and Healthy Ageing Strategy. The position focuses on early intervention activities and models of care that support healthy ageing and the prevention and management of chronic conditions. The position also works to support after-hours access to primary care for residents in residential aged care facilities and the community more broadly.

Key Responsibilities

  • Commissioning early intervention and after-hours access initiatives that promote healthy ageing and the prevention and management of chronic conditions in alignment with the CESPHN Older Persons Needs Assessment and Healthy Ageing Strategy.
  • Management of aged care and after-hours contracts (monitoring the progress and outcomes of commissioned activities and reporting on performance).
  • Increasing awareness in the community of local primary health care services that support the health and well-being needs of older people in the CESPHN region.
  • Assisting with the delivery of aged care related education and training to build capacity and capability in the local primary health care workforce to deliver high quality care to older people.
  • Assisting with the development and implementation of after-hours planning to support residents in aged care homes to access the most appropriate medical services out of hours.

Selection criteria – essential

  • Tertiary qualifications in a health-related field and/or relevant experience.
  • Experience with commissioning services to address needs and gaps in health care.
  • Experience managing the implementation of primary care and community-based health programs and projects.
  • Excellent organisational skills.
  • High level written and verbal communication skills.
  • Experience with common computing applications, e.g. Excel, Word, Database, SharePoint.
  • Possession of a current NSW drivers license and access to a motor vehicle.
