Your opportunity

As a Care Partner Support Officer, you’ll play a vital role in supporting the Care Partner team through administration, coordination, and client caseload management.

Acting as a primary point of contact, you will provide exceptional customer service while supporting the delivery of care under the Wellness Model of Care.

You’ll make an impact by

• Acting as a key contact for clients, families, and advocates
• Supporting Care Partners with coordination, scheduling, and administrative tasks
• Managing a client caseload while monitoring wellbeing and outcomes
• Maintaining accurate, timely documentation across systems
• Collaborating with the wider team to ensure continuity of care and positive client experiences

What you’ll bring

To succeed in this role, you’ll need the following qualifications:

• Diploma in Community Services Coordination, Certificate III in Aged Care, or equivalent
• National Police Check
• Current Australian Driver’s Licence

We would also love you to bring the following skills:

• Strong relationship-building skills and a genuine ability to connect with clients and families
• Empathy and respect for people from diverse backgrounds
• Excellent communication and customer service skills
• Strong organisational skills and attention to detail
• The ability to manage competing priorities and meet deadlines
• A collaborative mindset and willingness to support team outcomes
• Initiative and a proactive approach to problem-solving
