About the role

Community Health provides a range of allied health and nursing services including planned activity group. The social support coordinator role encompasses the planning, delivery and continuous evaluation of of social support activities with older community members such as centre-based activities and day trips. This role also includes providing leadership and support to a team of social support workers.

About you

To be successful in this role you will possess:

  • Demonstrated experience in the planning, delivery and evaluation of social support groups and activities.
  • Strong understanding of the importance of social connection and its link with health and wellbeing, especially in the ageing population.
  • Post Graduate qualifications in community services (aged care and/or leisure and lifestyle)
  • Highly developed interpersonal and leadership experience
  • A strong commitment to high quality care
  • The ability to work as part of a diverse team
  • Excellent communication and organisational skill
