Role description

The At Home Nurse provides clinical care to clients in their homes across Mount Alexander Shire, supporting safe alternatives to hospital-based care. This role works within programs such as Hospital in the Home (HITH), Geriatric Evaluation Management (GEM) at Home, Residential in Reach (RIR), and Rehabilitation in the Home (RITH), available seven days a week.

Key responsibilities include delivering high-quality nursing care, supporting health education, coordinating with multidisciplinary teams, and assisting clients through all phases of care—planning, monitoring, and transition. The role also involves recognising clinical deterioration and ensuring seamless care transitions.

This position supports Dhelkaya Health's value-based care approach by improving client outcomes, enhancing care experiences, and strengthening service flow between hospital, community, and residential settings.

About you

  • Bachelor of Nursing with current AHPRA registration and at least 2 years full time equivalent working as registered nurse
  • Sound understanding and experience in delivery of acute and sub-acute nursing care pathways across both inpatient and community settings
  • High level communication and interpersonal skills with the ability to establish positive working relationships with clients, carers and colleagues
  • Current Victorian drivers' licence
