Country to Coast is a community-based organisation supporting people over the age of 65 on Government funded Home Care Packages, with a range of in-home care services.

As our business grows we have a fantastic opportunity available for a qualified Care Coordinator.

About the role

  • Full time, Monday to Friday 9-5pm with one on-call weekend every six weeks.
  • Mostly work from home and occasional on-site office in Anglesea, Victoria.
  • Above award wages at $42 per hour.
  • Paid travel allowances.
  • Great working conditions and team.
  • You will be overseeing two staff with low caseloads.
  • The position purpose is to support consumers to remain living independently within their community and to deliver quality care and services to individual consumers as scheduled and set out in their plan of care and services. Care plans and the delivery of service include, but are not limited, to domestic care, personal care, respite care, community nursing and allied health services.

Key Selection Criteria – Essential

  • Registered Nurse – AHPRA or relevant Health Qualification
  • Experience in aged care setting or community care
  • Ability to make assessments of consumers with complex conditions and work collaboratively in a dedicated team environment working with office, care, and nursing staff for best consumer outcomes
  • Demonstrated ability to oversee 2 x Care Partners activities
  • High attention to detail in administration, strong computer literacy and ability to prioritise workload in a faced paced and dynamic environment
  • Demonstrate skills, knowledge, attitudes, values, and abilities that underpin safe and effective care
  • Act as a consultant upon request/provide education in own area of expertise
  • Work as part of a team with a flexible and responsive approach
  • Commitment to “person centred” care
  • It is a mandatory requirement that we will require a National Police Check prior to commencement
  • Completion of a Statutory Declaration form – no convictions or assaults
  • Excellent communication skills verbal and written
  • Well‑developed planning and time management skills
  • Current driver’s license
  • Have the use of a registered and fully insured motor vehicle that is suitable for consumer transport if required
  • Occasional in-service component – may be required to perform duties such as personal care or transport
  • On-call component one weekend – six weekly.

Key Selection Criteria – Desirable

  • Understanding of the need of aged care consumers
  • Some knowledge or understanding of Dementia
  • Experience with E-HCP software – e-Tools
