About you and the role

We are looking for a person to join our team our Support Coordination team and take on an existing caseload of participants, helping us grow the Community Gateway brand within the Support Coordination space. You will be a self-starter and able to hit the ground running with support from your manager and colleagues. Your responsibilities include:

  • Demonstrate a proactive approach to assessment, planning, review and delivery of support options that are spirituality and wellness focused and build on the strengths, capacity, goals and interests of clients
  • Coordinate, review and monitor the delivery of supports and services identified in the client plan
  • Creatively address barriers and encourage actions that promote a level of independence, reduce risk and ensure people remain well for as long as possible
  • Maintain accurate and comprehensive records, in accordance with policies and procedures, including client records
  • Provide mentoring and peer support to other less experienced Care Managers
  • Ensure adherence to all legislative and regulatory requirements

Essential Criteria

  • A minimum of three years of full-time experience, or an equivalent number of hours in a Case Management position
  • Understanding of, and commitment to, person-centred and strengths-based practices 
  • Thorough knowledge and understanding of the NDIS
  • Demonstrated ability to provide mentoring and peer support to other less experienced Care Managers 
  • Relevant tertiary qualifications at a minimum certificate IV
  • Superior customer service and engagement skills, with the ability to continuously build a strong client base and develop solid relationships with existing and potential clients 
  • Demonstrated experience using Microsoft Office Suite and Client Management Systems 
  • Current unrestricted NSW Driver's Licence
  • A current Working with Children Check and current NDIS Worker Screening Check

Desirable

  • Certificate IV in Aged Care or other relevant qualification
  • Knowledge of local networks
