We have an exciting opportunity to recruit to our vibrant team at HART 4000 for a part time and full time role paid at Level 4 SCHCaDS.

The Homelessness Intervention Worker will be part of a dynamic team based at HART 4000 office in Fortitude Valley.

This full time position aims to connect, refer and communicate with relevant support agencies. It will involve in-house support and active referrals to assist people to link with the services they need.

WE ARE LOOKING FOR SOMEONE WITH:

• Social Work degree and/or extensive experience in human services
• Demonstrated skills and knowledge of community based intake, assessment, case management & documentation procedures
• Demonstrated excellence in interpersonal, written and organisational skills
• Ability to establish & develop links with communities, communicate & liaise with service providers & work with complex needs clients to provide good case management
• Current valid drivers licence
• Current Police Check
