About the role

The primary focus of the role is for the Mental Health Nurse under the Australian Governments' Department of Health's Mental Health Nurse Program (formerly known as the Mental Health Nurse Incentive Program) to assist in the implementation of a coordinated responsible service that provides support to the General Practitioners in managing clients with severe and complex mental illness in the Western regions of Melbourne.

The Mental Health Nurse is expected to perform duties within the scope of practice of the role, and according to the clinical privileges confirmed through the verification of Mental Health Nurse credentials, qualifications, registration and professional competencies.

The Mental Health Nurse is expected to develop and maintain collaboration with the North Western Melbourne Primary Health Network which coordinates the MHN Program, including the Intensive Support Service (ISS) and Targeted Psychological Services (TPS, formerly known as ATAPs)

Your daily duties will vary but will include:

  • Undertake practice in accordance with the Australian Health Practitioners Regulation Agency
  • In collaboration with psychiatrists, General Practitioners and relevant health services and agencies, Mental Health Nurses provide coordinated services to clients who meet the MHN Program eligibility and live within the North Western Melbourne Primary Health Network areas
  • Develop and maintain collaboration with key external stakeholders including area mental health services, tertiary hospitals and aged care assessment teams.
  • Provide client treatment and review in accordance within MHN goal directed care plans and GP mental health plans.
  • Provide Mental Health Nursing consultancy to cohealth staff.
  • Ensure standards and legislative requirements are met and maintained whilst managing the clinical environment in the areas of:
  • Infection Control
  • Accreditation
  • Occupational Health and Safety
  • Report to Medical Practice Manager to provide monthly completion of income targets in conjunction with the PHN's Intensive Support Services (ISS) and Medicare's Targeted Psychological Services (TPS), formerly ATAPs, along with other income generating opportunities that may arise.

About you:

  • Registered Nurse Division 1 with current registration with AHPRA (Victorian Nurses Board) with minimum of 3 years postgraduate experience
  • Credentialed with the Australian College of Mental Health Nurses
  • Professional Indemnity cover
  • Experience and knowledge of mental health programs and an understanding of funding models to maximise opportunities for clients, with experience in meeting monthly targets and KPIs
  • Ability to work autonomously and as a part of a team and to work within limited time frames
  • Demonstrated client-centred approach in service provision
