About the role

This position sits within the AOD and Homelessness Cluster which delivers value-based care and aims to improve the health, wellbeing, housing, and social outcomes of people who use alcohol and other drugs and/or those experiencing homelessness in the Inner North West of Melbourne.

The AOD Recreation and Inclusion Worker will support the development of social inclusion, psycho social assistance and recreation support services with moderate to severe mental ill health, complex physical health, substance use and social support needs who face significant barriers to accessing services. The support provided to the client group includes engagement, care planning with tailored psychosocial and health supports to individuals with the aim of promoting physical activity whilst increasing social and community links for participants.

Your duties include:

  • Participate in outreach activities
  • Establish and maintain links with other key local agencies, local community and provide secondary consultations as required
  • Develop and implement client-directed, strengths based individual client care plans and service packages
  • Participate in the leading recreation and inclusion programs within the team and adopt the role of role model and mentor with consumers by leading by example

Our ideal candidate:

  • A relevant tertiary qualification, experience in the coaching of a sporting/recreational activity, housing, mental health, AOD, disability or welfare sectors, a lived experience is highly valued
  • Demonstrated experience in working with clients who have a history of homelessness and multiple and complex needs, as well as a sound understanding of the key issues affecting people experiencing multiple and compounding disadvantage
  • An understanding of the Social Determineates of Health and Trauma Informed Care Practices
  • An understanding of the principles of consumer directed care, care planning, advocacy, and referral
  • The ability to establish respectful professional relationships that have clear boundaries with consumers, staff, and partner organisations
