We are currently seeking a Social Emotional Wellbeing Worker will support community with their emotional and spiritual well-being including mental health concerns through counselling, family support, referral and advocacy. 

Key Selection Criteria:

  • Demonstrated understanding and commitment to Victorian Aboriginal health, local Aboriginal culture, and the philosophy and practice of Aboriginal Community Control health service.
  • Demonstrated ability to work with Aboriginal organisations, communities and individuals in culturally appropriate ways 
  • Demonstrated understanding of the diverse health and social needs of Aboriginal people
  • Well-developed interpersonal skills, including a friendly and professional approach and an ability to deal with a broad range of people at all levels.
  • Sound written verbal and communication skills
  • Demonstrated organisational and time management skills, including an ability to prioritise, manage multiple tasks, work to deadlines, manage interruptions and adapt to changing priorities
  • Demonstrated capacity to manage sensitive information, maintain confidentiality and remain impartial at all times. 
  • Self-motivated and demonstrated ability of working independently with minimal supervision, and as an effective team member, promoting cooperation and commitment to achieve goals.
  • Demonstrated proficiency and experience using Microsoft applications (e.g. Microsoft Word, Excel, Outlook) combined with a high level of accuracy and attention to detail.
  • Previous experience in community support role and/or formal qualification relevant to role.

Duties include (please see position description for full details):

  • Actively promote and provide a holistic approach to care of the Aboriginal people around social and emotional wellbeing.
  • Perform emotional, social and mental health assessments of clients and their families.
  • Provide case management and care coordination for individuals and their families including coordinating with other services.
  • Liaise and advocate with visiting General Practitioners and other health professionals.
  • Advocate for individuals, families and the Community on social, emotional, wellbeing and health issues.
  • Develop and maintain a therapeutic relationship with clients and their families and the community.
  • Utilise key resources and coordinate with external agencies to ensure clients and their families can access the services and supports they need.
  • Promote individual and Community social and emotional wellbeing through 1:1 and group activities.
  • Identify and seek funding from external agencies / government to implement programs, projects and activities which address the social and emotional wellbeing needs of the Community.
  • Provide support and advice to external services/health professionals to ensure they provide culturally appropriate services to the Community.
  • Actively uphold CHADAC philosophy, policies and procedures. 
  • Maintain confidentiality and accurate client records. All client history, conditions, procedures, referrals, recalls being recorded in Communicare and progress notes to be written. 
  • Maintain professional skills and knowledge via appropriate staff development activities.
  • Participate in quality improvement activities that maintain and further develop a responsive and organised AMS, in particular accreditation.
