The Opportunity

CF Together is committed to the provision of timely, effective, person-centred, and evidenced based programs and services to people living with CF, and to raising awareness about CF with a broad range of stakeholders.

As part of a national team, the Case Manager will assess, review and support cases referred to CF Together.  Once assessed, the Case Manager is to provide a range of counselling, practical support and social work services to adults and families living with CF.  Extensive case management support, outreach, engagement and assistance is provided remotely by phone, e-mail, teleconferencing, and on occasion, community support events. 

This role will also support the capacity of the wider Services Team and will provide high quality customer service to current and prospective CF Together members. 

Key Responsibilities

  • Counselling, support, education and advocacy for adults and children living with CF and their families
  • Provision of grief and loss, personal-centred, and skills-based counselling, psychosocial assessment, and time limited psychotherapy
  • Advocating for those impacted by CF in hospital and community setting
  • Assistance with income support entitlements, referrals to social and community support services
  • Liaising with hospital staff as appropriate, sometimes as a member of the multi-disciplinary team
  • Meeting monthly and quarterly KPI targets for service provision
  • Assistance with access to internal CF Together programs and external programs (e.g. NDIS, Centrelink)
  • Assist with the delivery of our programs and support services, processing membership requests, processing applications for support, assisting with support activities and responding to supporter queries
  • Maintain data entry requirements by following CF Together processes and guidelines around privacy, security and data tracking

What will you bring to the role?

  • Tertiary qualifications in counselling, psychotherapy, psychology, social work, community work or similar
  • Professionally registered membership with PACFA, ACA (Level 2 and above), AASW, ANZACATA or equivalent
  • Experience in counselling or a social worker role and the ability to demonstration a professional work history. Health experience is an advantage
  • Ability to communicate in a positive and effective manner with clients, hospital clinicians, members and supporters of CF Together, volunteers, colleagues, and representatives of other organisations
  • Ability to exemplify health-promoting practices to people with CF including responsible self-management of stress and fitness, and to reflect a healthy approach to respiratory wellbeing
  • Effective personal organisational skills and the ability to perform in an autonomous role
  • Ability to maintain up to date and accurate client files and program records, write professional reports, and provide relevant documentation to other stakeholders
  • Willingness and ability to comply with the policies of the organisation, with privacy, confidentiality, health and safety policies, and working with children
  • Experience working in multi-disciplinary teams or with external stakeholders advantageous
  • Experience of knowledge of the challenges faced by people with CF or other terminal chronic disease, or invisible illness is an advantage

Highly regarded:

  • Experience with funding programs and grants, which require meeting specific contractual obligations and deliverables
  • Experience with telehealth and telephone-based services is a plus

This position is a part time (0.8) fixed term contract (12 months) and can be worked remotely, or hybrid, if based in Melbourne. CF Together aims to support flexibility for all staff while maintaining collaboration and productivity to achieve organisation objectives.
