An excellent opportunity exists for a Community Health Nurse (CHN) to work within a harm minimisation framework to deliver health services to the residents of a Launch Housing emergency accommodation complex, located at South Bank, Melbourne. This is a 51-bed facility offering mixed crisis accommodation for adults.

Services are delivered via a nurse operated clinic on site and include comprehensive health assessments and clinical nursing care such as wound, diabetes management, pathology, immunisations and so forth.

In addition, the nurse conducts health promotion and educational sessions targeting the population residing within the facility. There is a focus on health advocacy, support for clients to attend appointments, liaising with relevant service providers to improve client health outcomes and independence. There is a strong partnership with Star Health/Better Health network and their GP and Dental clinics and other relevant services.

Integrated Complex Care Program) is funded by the South-eastern Melbourne Primary Health Network and is responsible for providing highly flexible, timely and accessible support to consumers with severe mental illness and complex needs in the community

Ideally, you will possess:

  • A professional qualification as a Registered Nurse, Division 1 with current AHPRA registration.
  • A minimum of three years' of post graduate experience, preferably including a community context.
  • Demonstrated ability to deliver Div1 nursing care eg. immunisations, venepuncture, wound care, diabetes, care coordination, health promotion, infection control and emergency response.
  • Ability to work holistically, including ability to coordinate care plans with multiple stakeholders.
  • Provision of community health nursing people with complex needs.
  • Excellent knowledge of working with clients with serious mental health and substance abuse issues.
  • A clear understanding of the needs and issues affecting clients experiencing homeless or at risk of homelessness and their complex co morbidities that are barriers to accessing care.
