About the Role

BHN is seeking a Community Health Nurse to join our experienced and remarkable Community Health Nurse team based at St Kilda location and working across Port Phillip, Glen Eira and Stonington municipalities. Be part of a broader team of muti-disciplinary professionals (Social Workers, Allied Health, Case Managers, Indigenous case workers).

As the Community Health Nurse your responsibilities will include

• Manage a diverse client caseload for people aged 25 and over with complex health needs.
• Outreach Home visiting model
• Client service coordination and advocacy
• Utilize clinical nursing skills and assessment to improve client health outcomes.
• Multidisciplinary approach managing a complex caseload with an effective throughput of cases

About you

• Tertiary qualification in Division 1 Registered Nursing with minimum 3 years RN experience
• Community sector and service coordination experience with advocacy for complex clients is preferable
• Experience working with My Aged Care, NDIS, ACAS, HACC-PYP, RAS, CHSP
• Relevant Post Graduate experience
