As a Case Manager in our Community Connections Program, you'll engage with vulnerable individuals experiencing homelessness or insecure housing. Join our dedicated team to provide vital support and assistance.

Duties and Responsibilities:

  • Conduct assertive outreach to engage with homeless individuals
  • Screen referrals and assess eligibility for the program
  • Provide case management and link clients to support services
  • Advocate for clients at individual and systemic levels
  • Maintain accurate client records and participate in networking activities

Requirements and Attributes:

  • Tertiary qualification in Social Work or equivalent
  • Experience in outreach services and complex case management
  • Understanding of social determinants of health and homelessness
  • Excellent communication and interpersonal skills
  • Unrestricted Victorian Driver's License
