Our Dementia team is looking for a passionate and highly experienced Clinical Manager with clinical experience in Dementia and Chronic Disease in a Community Health setting to support our team on a permanent 0.8FTE basis. Flexible hours to suit your lifestyle, with no ongoing late nights or weekend work required. Hybrid working model from our Parkdale head office, home and client visits within the service area. Access to a company car for home visits.
In your role, you will be responsible for managing 30-40 clients according to your working style. Clients and carers are supported in the community by our team, making a real difference to their lives.
Since our multidisciplinary Dementia Support team has worked together for over 15 years, we have accumulated a wealth of knowledge, networks, and processes. You will be part of large community health organisation with access to a variety of allied health services and other disciplines.
Our service aims to assist people with cognitive decline and/or dementia to remain living independently at home.
We have excellent relationships with other local services and work in a coordinated approach with regional assessment service, aged care assessment service, government/non-government organisations and local council. The team meets regularly for cases discussion and peer support.
