The position is offering an opportunity to support those who are aging within the community to remain living independent in their own homes and active within their communities.
CatholicCare's Seniors in Networking Communities (SINC) Program, is looking for a dedicated Social Worker to join our team in delivering Commonwealth Home Support Funded Allied Health Services to seniors living in their own homes in the community.
This service delivers social work services to older people throughout areas of NSW, working both with individuals and with groups to assist older people to regain or maintain their independence.
The role offers a variety of work environments, with interventions taking place in individual's homes and in the wider community.
The key responsibilities for the SINC Social Worker will be to work with older people who have been referred by My Aged Care to the SINC Program for Allied Health Services. Our SINC Social Workers focus on, person centred, wellness and reablement and work within a restorative care framework providing individualised support, counselling, and interventions to assist people with being able to remain in their homes and as part of their community.
This role includes some long distant travel and possible overnight trips for work and training purposes. With the programs administrative team being based in Canberra, a level of comfort with travel and virtual meetings is required.
