About the Position

Key Responsibilities
- Provide clear and comprehensive information about the NDIS, funding utilisation, and available services.
- Identify and connect participants with suitable services, ensuring their preferences and needs are met.
- Empower participants to make informed decisions about their support, fostering independence and autonomy.
- Assist with service agreement execution, budget management, and financial advocacy.
- Facilitate opportunities for social participation and community integration.
- Provide timely assistance during challenging periods, such as hospitalisations or housing transitions.
- Prepare detailed and accurate reports to support plan reviews and service delivery.
- Adhere to NDIS Quality and Safeguards Commission standards.
- Excellent communication and interpersonal skills.
- Strong organisational skills and attention to detail.
- Empathy and compassion towards clients and their families.
- Knowledge of healthcare systems and resources.
- Previous experience in a similar role is a plus.
- Ability to work independently and as part of a team.
Pay Rate
The pay rate for this position is competitive and will be discussed during the interview process based on experience and qualifications.

Qualifications
- Two years Support Coordination preferred.
- Two years of work experience within the NDIS Sector/industry and should be knowledgeable about the procedures and policies within NDIS.
- A minimum of a Certificate IV/Diploma in Disability, Community Services or a related field.
- Proficiency in Microsoft Office and client management software.
- Current driver’s licence with reliable and comprehensively insured car.
- Working with Children’s Check.
- NDIS worker clearance.
