About the role

The Program Manager will primarily focus on the Older Persons Health Planning Program. You will be responsible for the needs analysis, development of effective proposals, planning and implementation of activities to deliver better health outcomes for older persons living in the ACT.

As part of this role, you will use your project management skills and understanding of the issues impacting on the health and wellbeing of older persons.

About the team

The Health System Improvement (HSI) Business Unit comprises of two teams.

  • Health System Integration
  • Quality Improvement.

The HSI Unit is responsible for coordinating projects and program areas centred around improvement and integration of the health system, and implementing quality improvement activities to actively respond to the needs of Canberrans. The Program Manager Older Persons Health Planning sits within the Health System Integration team.

Reporting to the Senior Manager, HSI you will be responsible for supporting the development and management of system integration activities across key stakeholders, to support healthy ageing activities. You will lead the development of CHN’s Older Persons Strategy. You will develop collaborative external stakeholder relationships. Working closely with our Service Relationship Coordinators, you will support the commissioning, development and initiatives in healthy ageing and ongoing management of chronic conditions.

As part of working at an innovative organisation, you will also contribute to process improvement and broader Capital Health Network (CHN) projects and initiatives. You will have an opportunity to work closely across all Business Units within CHN.

Work location

This position is based in our Deakin office. We support flexibility and a hybrid work model at (CHN) and are happy to work with the successful candidate to ensure balance between work and life.

Salary: $107,179 plus superannuation
