About the Opportunity:

Broome Regional Aboriginal Medical Service (BRAMS) is seeking an enthusiastic Aged and Disability Social Worker who will use their expertise to provide individualised support to people with disability and frail aged people. The Social Worker helps consumers to understand what is included in their NDIS plan and/or Aged Care package, supports consumers to identify goals and needed supports and to choose and connect to suitable support services.

The role requires knowledge and understanding of the aged and disability services sector and service standards, human rights, and the principals of person centred plans, and the concept of individual choice and control.

The Broome Regional Aboriginal Medical Service (BRAMS) is an Aboriginal Community Controlled Health Service which has been caring for the Broome community for more than 40 years. When we first opened our doors in 1978, BRAMS was the first remote Aboriginal Medical Service in Western Australia.

BRAMS provides comprehensive, holistic and culturally responsive primary health care, social and emotional wellbeing services, and NDIS support to Aboriginal people living in Broome. BRAMS delivers more than 40,000 occasions of service each year. BRAMS delivers services in accordance with our Model of Care.

Our mission is to provide holistic and culturally responsive health and wellbeing services for Aboriginal and Torres Strait Islander People. That means making our Mob healthy.

Key Responsibilities:

Some of your key tasks will include:

• Conduct person-centred assessments for each participant and collaborate with the consumer to develop comprehensive plans aimed at helping them achieve their individual goals.
• Provide case management services to allocated consumers, delivering the specified number of hours and agreed-upon outcomes.
• Offer necessary support to each consumer based on their individual needs.
• Evaluate the services and resources provided to consumers to determine their effectiveness and modify consumers' plans and recommendations as required.
• Maintain regular follow-up with consumers to ensure their ongoing needs are met.
• Maintain appropriate case notes and other relevant documentation on participant records.

What you will bring to the team:

To be successful in this role, you will have the following:

Essential Criteria

• Bachelor’s Degree in Social Work, plus minimum 1 year work experience.
• Registration or eligibility for registration with AASW.

Desirable Criteria:

• Demonstrated knowledge of current issues, standards and trends in the delivery of mental health and social and emotional wellbeing services to Aboriginal people.
• Demonstrated experience or understanding of health and wellbeing issues affecting Aboriginal and Torres Strait Islander people, including a demonstrated awareness of and sensitivity to Aboriginal culture and history.
• Excellent interpersonal skills with the ability to work with a broad range of people from a variety of backgrounds and experiences as well as highly developed verbal and written communication.
• Demonstrated ability to maintain absolute confidentiality regarding patient and practice information.
• Demonstrated computer proficiency, including the use of Microsoft Office applications, clinical record and data management systems and software, as well as proficiency in report writing and demonstrated ability to develop, organise and maintain records and reports in a timely manner.

About the benefits:

• Base salary of $95,000 to $110,000
