About the Role:
Part time position (20 hours per week), Kirrawee location.
As a Case Manager, you'll be the heart and soul of our customer experience! You'll connect with prospective residents, clients, families, and advocates, ensuring they feel valued and supported. Your expert assessments will maximise services, working hand-in-hand with both internal and external partners. You'll make sure our clients' clinical needs and desires are met through smart and effective case management. Plus, you'll help grow Seniors Living by highlighting what makes us unique and seizing new opportunities!
What You'll Do:
- Client Management: Provide top-notch customer service, assess client needs, and create personalized care plans.
- Smooth Transitions: Guide clients from initial contact to the start of care, building trust and credibility.
- Proactive Care: Regularly assess and advocate for clients to maximize their services and support.
- Documentation: Keep accurate records in our CRM and IT systems.

What We're Looking For:
- Diploma in Service Coordination/Community Services or equivalent.
- Exceptional customer service and sales experience.
- Aged care sector experience, especially in home assessments.
- Tech-savvy with a current driver's license.
