About the Role
Are you passionate about making a difference in the lives of older people? Join our team as a Care Finder Support Worker and provide specialist assistance to help people within the care finder target population understand and access aged care, and connect with other relevant community supports.
As a Care Finder Support Worker, you will play a crucial role in supporting older adults to navigate the aged care system and access the services they need. Your compassionate approach and dedication will help clients feel understood and supported throughout their journey. You will engage with clients through assertive outreach, build strong partnerships with key professionals, and ensure that clients receive timely and effective assistance.
Maximum Term, 12 month, Full Time, located in the Upper Blue Mountains Monday to Friday 830am to 430pm or 9am to 5pm work hours.
Specific Accountabilities
- Outreach: Engage with potential clients and intermediaries through outreach activities, building meaningful connections.
- Partnerships: Develop and maintain partnerships with key professionals to identify and engage target groups.
- Enquiry Management: Promptly action general enquiries and intake applications, building rapport and trust with potential clients.
- Service Access: Assist clients in interacting with My Aged Care for registration, eligibility, and referral for assessment.
- Assessment Support: Support clients through the assessment process, including form completion and attending assessments.
- Outcome Achievement: Identify and support connections with appropriate services to help clients achieve their goals.
- Ongoing Support: Provide periodic check-ins and assist clients in connecting to new providers if circumstances change.
- Client-Centric Approach: Ensure client choice, control, safety, and welfare drive all actions and decision-making.
- Documentation: Complete and maintain appropriate documentation and data in CRM.
- Caseload Management: Maintain a full caseload within organizational guidelines.
- Professional Development: Attend regular supervision, relevant training, monthly meetings, and other professional development activities.
- Program Improvement: Contribute to the continuous improvement of the program by refining policies, procedures, and work practices.
- Safety Culture: Maintain a safe, clean, and tidy workplace, comply with safe work practices, and report hazards and incidents.
- Organizational Understanding: Maintain a comprehensive understanding of Anglicare's strategy, vision, values, policies, procedures, manuals, guidelines, and reporting requirements.
Education and Experience
- Relevant tertiary qualifications in Community Services/Welfare/Aged Care/Social Work, or equivalent relevant qualification.
- Previous experience in Home Care/Community/Aged Care or related services.
- Exceptional interpersonal skills in communication, negotiation, and problem-solving.
- Current NSW driver's licence and vehicle.
- Intermediate computer skills in MS Office.
