Your Opportunity

The Transitional Youth Support Service (TYSS) is an assertive outreach program for young people aged between 16-25 years who are homeless or at risk of becoming homeless. TYSS supports young people to strengthen their independence by providing support to achieve long term housing goals, build on living skills and links to community during tenancies in transitional accommodation or via outreach in a community context.

As the Case Manager in the TYSS team you will support young people who are transitioning in and out of the program as required and work collaboratively with the broader team to provide a high standard service delivery to homeless or at risk of homelessness young people.

Your skills and experience

We are looking for an experienced case manager that is able to provide a high level of support and case management to young people experiencing homelessness. You will have a passion for making a change and improving the living standards for young people who are experiencing homelessness.

Our ideal candidate will promote a positive culture within the team and support their team members, as well as demonstrating the ability to build positive external stakeholder relationships.
