About the Role:
As a Support Coordinator, you will be responsible for providing high quality support coordination for NDIS participants based on their Individual Support Plans and service agreements. They are a client-centred organisation and are seeking an experienced NDIS professional who is passionate about supporting vulnerable clients.
In this role, you will:
- Lead support coordination for an assigned caseload, ensuring participant satisfaction and retention.
- Assist participants in developing their understanding of the NDIS and building connections with the community and systems of support.
- Maintain regular contact for goal renewal, service provisions, and spending with participants.
- Provide services in alignment with participants’ NDIS plans and service agreements, meeting all specified needs.
- Build trusting and lasting relationships with participants, aiding them through challenges or crises as well as the day-to-day.
- Collaborate with the NDIS and key stakeholders to ensure participants receive eligible entitlements.
- Work internally to meet financial, employment, and support worker needs, supporting plan renewals and management activities.
A Bit About You:
To have success in the role, you will be a adaptable and passionate NDIS professional with the ability to deliver person-centred services. You will also have:
- Proficient understanding of the NDIS and knowledge of relevant legislation, regulations, policies, and practices.
- Prior experience in a similar Support Coordinator position or a role in Disability Services.
- Effective caseload prioritisation and daily management skills.
- Strong interpersonal and stakeholder engagement abilities, using a participant-led approach emphasising collaboration.
- An understanding of budgeting and financial management
- Proficiency in Microsoft Word, Excel, and Outlook
- An unrestrictive Driver’s License, NDIS Worker Screening Check, and Working with Children’s Check (WWCC).
