About the Role

Our Care Consultants have experience with Case Management / Care Plans. You'll be visiting our clients in their homes located in Bendigo, Seymour and surrounding suburbs, to conduct assessments, care planning and ongoing client management.

Key Duties

• Initial Customer assessments & service planning
• Work with your customer to understand what they want from their Home Care Package, and assist them to build it based on their level of funding
• Care planning, regular monitoring, and reassessment of customer needs
• Regular visits to customers in their homes  
• Providing ongoing case management to HCP and/or private customers

About You

You are passionate about continuing to grow your career in case management. Working as part of a close-knit team, you collaborate with and support others, demonstrating respect in your day-to-day actions.

This opportunity is working from home. You will be in the field 70-80% of the day, visiting customers.

You will ideally bring the following:

• Qualification in Case Management, or Undergrad qualification in Nursing / Allied Health / Social Work is essential
• Div 1 Nursing Qualification - desirable but not essential
• Prior case management experience will be highly regarded
• Experience completing initial and annual customer assessments and care planning
• Knowledge of the Home Care Package industry

Mandatory:
