Ballarat Hospice Care Inc. (BHCI) is a home-based palliative care service working in the community; caring for people living with a life limiting illness and supporting their family and carers in their place of residence.

We are a team of specialist palliative care nurses, community service professionals, grief and bereavement counsellors, patient and family support volunteers and compassionate administrative staff. We work closely with GPs, local hospitals, specialist doctors and nursing services – all to meet the varying and individual needs of the people we care for including 24hr support and loan of equipment without direct cost to the patient or family. We also offer education to carers and patients and to health and community services and community groups. We cover the Hepburn Shire, City of Ballarat, the northern end of the Golden Plains Shire and the western end of Moorabool Shire.

With an increase in service demand and a brand new palliative care hub just about to be opened, Ballarat Hospice Care Inc. (providing in-home palliative care) is seeking experienced and qualified staff to join its highly regarded team.  

Ballarat Hospice Care Inc. provides community palliative care that is patient and family centred and is offered through a multidisciplinary team, addressing physical, psychosocial and spiritual needs. The BHCI team are excited to be soon in their new ‘Hub’ which has been designed to  create the right atmosphere for on-going professional development, team building, liaison with other like-minded services and to offer increased choice in how we care for people, each other and our community.

Key Responsibilities

  1. Demonstrates and upholds the values of BHCI through professional attributes and behaviours
  2. Undertake psychosocial and spiritual assessment, to identify the needs of the patients and their carers / families and collaboratively, with SCT, plan and evaluate care
  3. Effective and comprehensive communication / clinical handover to the BHCI team and members of the wider healthcare team regarding the patient / carer needs
  4. Facilitate patient and family support groups with the aim of promoting a safe space to share common life experiences in which to build resilience and capacity for all participants
  5. Demonstrate effective and efficient management of own workload to include opportunities for research and quality improvements
  6. Meaningful self-care, professional self-development and participate in regular supervision
  7. Utilise culturally appropriate intervention strategies for all patients and familiesincluding those with CALD, Indigenous, LGBTI background and vulnerable groups
