The Australian Childhood Foundation are looking for a Care Coordinator, who will support children and young people in the OOHC program by:

  • Collaborating with carers, families, and service providers.
  • Developing and implementing individual care plans.
  • Managing cases using a trauma-informed approach.
  • Facilitating external referrals as needed.

Undertaking some weekend events and after-hours roster work.

This program focuses on creating therapeutic environments for children who have experienced trauma, abuse, or family violence. By promoting healing, stability, and cultural connection, we aim to improve outcomes through holistic, evidence-based interventions and collaborative care.

In order for you to succeed, you will need (Key Selection Criteria):

  • A tertiary qualification in Psychology, Social Work, or a related field.
  • Experience in case management and working within the OOHC sector.
  • Strong knowledge of trauma and attachment frameworks.
  • Exceptional interpersonal and decision-making skills.
  • A current driver's licence, Working with Children Check (or equivalent) and able to undertake a national police check
