As a key member of the ATSICHS Brisbane team, the Youth Services Case Manager is expected to personally contribute to the shaping and achievement of ATSICHS vision and goals within their own scope. The Youth Services Case Manager will provide three (3) Key Functions.
The Case Manager provides tailored case management services for young people at risk of disconnecting with their family/community, disengagement from education, training and employment, homelessness, or at risk of harm.
Case managers are responsible for effective engagement with young people, their families and their support network to identify the young person’s needs and aspirations, and record and progress a plan created and led by the young person.
Case Managers act as the main ‘point-of-contact’ for the young person and their family, and work in partnership to facilitate supports required to achieve their goals. Case Managers develop, implement and execute the care plan of young people, and coordinate a wrap-around suite of services with the aim to achieve outcomes of the four (4) program objectives. Case Managers ultimately assist young people to realise increased self-worth, family and community connectedness, and safety, delivering a service underpinned by customer service principles.
Effectively manage and coordinate health and social support services to families, based on the young person’s Care Plan. Diligently and accurately record ongoing client information, case notes, and goal and outcomes tracking as part of high-quality case management practices. The Case Manager is a navigator of the health and social services system to improve access and quality of service responses to young people and their families.
The Case Manager works to coordinate and deliver interventions which encourage young people and their families in achieving their goals and provide crisis response care as required. The role is primarily delivered via a community outreach/in-home engagement approach to remove any access barriers to young people. The role leads internal and external case planning activities with other agencies and stakeholders where required. In addition the Case Manager acts as an advocate; to ensure young people are receiving quality services, are involved in all decisions that affect their lives, and are treated with respect, dignity and empathy.
Support fellow staff in a team environment to build collegiality and a sense of belongingness within the Youth Services team and ATSICHS family. Staff will actively participate in team activities and contribute to a flourishing workplace culture that promotes the ethos and values of ATSICHS Brisbane as a long-standing Aboriginal Community Controlled Organisation.
