Position Overview

The NDIS Support Coordinator is responsible for day-to-day service provision ensuring the highest quality support for the people who access the program(s), providing:

  • Direct Support Coordination to participants that have been identified as eligible for NDIS
  • Evidence-informed, strengths-based case management interventions for clients with an Acquired Brain Injury (ABI) and complex support needs.

About the role:

NDIS support coordination is a dynamic role involving a range of responsibilities. You will be responsible for:

  • Developing and implementing individualized NDIS support plans in collaboration with participants, families, and other stakeholders.
  • Coordinating and facilitating access to NDIS-funded supports and services according to participants' goals and needs.
  • Advocating for participants' rights and preferences within the NDIS framework.
  • Monitoring and reviewing participants' progress and adjust support plans as necessary.
  • Liaising with service providers, allied health professionals, and community resources to ensure coordinated and effective delivery of supports.
  • Providing guidance and assistance to participants and their families in navigating the NDIS system and understanding their rights and entitlements.
  • Maintaining accurate and up-to-date documentation and records related to support coordination activities.
  • Staying informed about changes and updates to NDIS policies, procedures, and guidelines.
  • Collaborating with interdisciplinary teams to promote holistic and person-centered care.
  • Fostering positive relationships with participants, families, and other stakeholders to promote trust and effective communication.

About you:

  • Possess a relevant tertiary qualification in social work, psychology, disability studies, or a related field, providing a strong foundation for understanding and addressing the needs of NDIS participants.
  • Demonstrate a proven track record of successful engagement in previous relevant roles, showcasing your ability to effectively support individuals with disabilities and navigate complex systems.
  • Exhibit in-depth knowledge about the NDIS and its inner workings, enabling you to provide informed guidance and support to participants and their families.
  • Display exceptional communication and interpersonal skills, allowing you to build rapport with diverse stakeholders, collaborate effectively within interdisciplinary teams, and advocate for participants' rights and preferences.
  • Show flexibility and adaptability in your approach, recognizing the unique circumstances and goals of each participant and tailoring support plans accordingly.
  • Possess strong organizational skills and attention to detail, ensuring accurate documentation and record-keeping and facilitating seamless coordination of supports and services.
  • Demonstrate empathy, compassion, and a genuine commitment to empowering individuals with disabilities to achieve their full potential and lead fulfilling lives.
  • Exhibit problem-solving abilities and a proactive mindset, enabling you to anticipate and address challenges, identify opportunities for improvement, and advocate for systemic change within the NDIS framework.
  • Embrace a person-centered approach to support coordination, prioritizing the autonomy, dignity, and well-being of participants and promoting their active participation in decision-making processes.
