About the Program

The Care Finder Program supports older people over 65 years of age, or over 45 years if Aboriginal or Torres Strait Islander, who are eligible for government funded aged care to learn about, apply for and set up support services.

About the Opportunity

The Care Finder / Aged Care Volunteers Visitors Scheme (ACVVS) Officer is responsible for the delivery of specialist and intensive assistance to support older people to access aged care services and other relevant supports in their local community.

In this role, you will provide assertive outreach services to connect and engage with people who meet the eligibility requirements for the Care Finder program, working with internal and external contacts to ensure access to services and maintenance of support required to live independently in the community.

Additionally, the role works across the Aged Care Volunteers Visitors Scheme, ensuring contract outcomes are met including the matching of volunteers and consumers.

About You

As the Care Finder / ACVVS Officer you will use your strong case management skills and practice to work with older people with multiple and changing needs including mental illness and homelessness You will bring with you your prior knowledge of aged care and homelessness to provide professional intensive support to participants. You will be an exceptional communicator, able to develop strong, professional relationships with both participants and external stakeholders.

Please note: It is a requirement that prior to commencing in this role, you must possess a Working with Children (Ochre Card), a National Criminal History Check (within 3 months)and a current Northern Territory driver’s licence.
