About the Program

Anglicare NT’s home support services help older people remain independent in their homes and communities longer. The service supports people aged 65 years and over and Aboriginal and Torres Strait Islander people aged 50 years and over who are experiencing difficulty with everyday tasks.

About the Opportunity

As the Care Coordinator you will coordinate supports and services for older people to help them stay in their own home as they age and to live happy, meaningful, and connected lives. You will be a an inspired and solution focused person, who is a good communicator and great at planning, record keeping, negotiating and logistics. You will work in a supportive team that understands the needs of older Territorians and is committed to quality care and continuous improvement.

To be successful in this role, you will have:

  • Relevant qualification or equivalent to Bachelor of Nursing (RN 1) / Allied Health / Social Work or Enrolled Nurse (RN2) / Diploma Community Services combined with Aged Care experience
  • Experience in the planning, co-ordination, and delivery of services to older people who live at home including care assessments and the development and review of care plans.
  • Knowledge of aged care and community services sector, regional service system and local providers
  • Demonstrated commitment to working respectfully with Aboriginal & Torres Strait Islander people and people from culturally and linguistically diverse backgrounds, communities and staff.
