Diversity, Inclusion and Cultural Competence

  • Ensure that the diverse needs of clients are considered and accommodated. This includes and incorporates physical, cultural, gender-identity and other.
  • Able to respect the diverse needs of other people.
  • Able to communicate effectively with people from diverse backgrounds.
  • In all instances works harmoniously with diverse people including people from CALD backgrounds.

Key Responsibilities

Clinical Service Delivery:

  • Conduct comprehensive functional assessments in clients’ homes and other community settings.
  • Identify barriers to independence and develop personalised therapy plans aligned with client goals and functional outcomes.
  • Prescribe and customise assistive equipment, home modifications, and adaptive techniques to enhance safety, mobility, and independence.
  • Provide skills training and education to clients, carers, and support workers on the use of equipment and activity modification strategies.
  • Deliver health education and reablement-focused interventions, including individual and group-based programs.
  • Support maintenance or restoration of skills necessary for daily living and community participation.
  • Participate in multidisciplinary care planning and case conferencing with allied health and external providers.

Documentation & Reporting:

Accurately prepare clinical documentation, reports, referrals, and care plans in accordance with organisational policies and best practice guidelines.

  • Complete funding applications (e.g., SWEP), discharge plans, and housing or environmental assessments as required.
  • Maintain up-to-date and timely client records in the Client Information Management System (CIMS), adhering to privacy and consent procedures.

Coordination & Collaboration:

  • Liaise with families, carers, GPs, case managers, and external service providers to ensure coordinated, integrated care.
  • Develop and maintain effective working relationships with internal and external stakeholders, including builders, suppliers, and community organisations.
  • Refer clients to other appropriate health services or programs as needed, ensuring timely discharge and follow-up support.

Professional Practice & Quality:

  • Ensure compliance with all relevant legislation, ethical frameworks, funding guidelines, and professional standards (including AHPRA).
  • Contribute to continuous improvement initiatives and participate in service development projects and program reviews.
  • Support and supervise Allied Health Assistants and students, where applicable, including planning and providing feedback for clinical placements.
  • Participate in operational and clinical supervision, professional development activities, and regular team meetings.
  • Engage in reflective practice and utilise feedback to inform ongoing skill development and clinical excellence.

Additional Responsibilities:

  • Contribute to a positive team culture and foster a collaborative, multidisciplinary approach to client care.
  • Support the delivery of culturally responsive services that respect the language, literacy, and beliefs of all clients.
  • Travel across metropolitan Melbourne to deliver in-home and community-based services as required.
  • Undertake any other reasonable duties as directed, aligned with the goals of the aged care and primary health program.

Key Selection Criteria

Essential:

  • Tertiary qualification in Occupational Therapy
  • Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Demonstrated experience working with older people and/or in community settings
  • Understanding of home modifications, equipment prescription, and functional assessment
  • Ability to work with clients from CALD backgrounds
  • Excellent communication and documentation skills
  • Current driver's licence and access to a reliable vehicle
  • National Police Check and NDIS Worker Screening (or ability to obtain)

Desirable:

  • Experience in aged care programs (e.g. CHSP, HCP)
  • Ability to speak a second language
