Due to our organisational growth and increase in our client numbers serviced through our Home Care Package (HCP) Program, we are seeking to appoint new Case Managers to join our team across Melbourne Metro.
As a Case Manager, you will provide high-quality case management services to our clients under the HCP program with a focus on enhancing their ageing experience and quality of life. As a service professional, you will be adept at building your relationships with clients and ensuring that all services are provided with adherence to the Aged Care Quality Standards, with a particular emphasis on ensuring that each client exercises their decision-making capacity.
Ideally, you would start before the festive season, however we are flexible for the right candidate.
You may be an experienced case manager with previous experience working with CALD and senior populations. Alternatively you may be experienced in the health/ allied health sector and seeking to move into case management, where we will support you with training and professional development.
You will be looking to join a diverse and inclusive multicultural organisation, perhaps enjoying the opportunity to engage with clients in a language other than English - we are particularly seeking candidates with Italian, Greek or Arabic as an additional language,
