About Us:
Established in 1982, Alzheimer’s WA provides care, support, awareness and information to assist people living with dementia, as well as their families and carers.
Our services include advocacy, education, partnerships, training, consultancy and holistic person-centred care programs, based on a philosophy of understanding the lived experiences of those living with dementia. We provide CHSP, HCP home care packages, day and overnight respite in our three homes based in Perth, Mandurah and Albany.

Position Summary:
We are currently seeking a highly motivated and experienced Clinical Nurse with demonstrated broad knowledge in all areas of clients care to join our organisation on a full-time basis.
The position will be responsible for visiting clients in their own homes. The Clinical Nurse role is primarily responsible for providing quality clinical care to those in the community living with dementia with clinical needs and supporting clients in achieving their goals and living independently in their homes. Be able to work independently and confidently to assess, monitor and review each client’s clinical needs.
Duties including but not limited to:
- Assist with the planning and implementation of clinical services which include but are not limited to: initial clinical assessment (PAS), Braden Scale, Mini nutritional assessment, planning, developing and implementing a clinical treatment and support plan, assessing falls risk and the home environment and review assessment of the client’s progress
- Deliver nursing care ensuring accordance with current ethical standards and relevant legislation.
- Wound evaluation and the assessment of wounds and dressings.
- Medication management and chronic disease management.
- Governance: ensure compliance with all contractual requirements, and service development in line with quality standards.
- Empowers and supports clients to advocate for themselves.
- Develops and promotes processes that encourage efficiency and effective teamwork.
- Be accountable and take responsibility for all care given including the assessment, clinical treatment and maintenance of care plan
- Undertaking medication competency assessments with Alzheimer’s WA Support Workers
As the successful Clinical Nurse, you will have:
Essential
- Clinical Nurse with at least 2 years’ experience
- Registered with AHPRA
- Experience in working with people with dementia in a range of care settings, such as acute, primary, community or residential aged care.
- Well-developed skills and expertise to provide person centred education and support to clients and carer’s in relation to their well-being and chronic health conditions.
- The capacity to problem-solve with clients, carer’s and families.
- Sound understanding of the Aged Care Standards and quality management systems
- A first aid certificate, completed less than 12 months ago
- Current and valid Police Clearance, WA driver’s license and own vehicle and willing to travel within designated area.
Desirable
- Knowledge of the broad range of services and referral pathways for people with dementia and their carers.
