What is the role?

  • 2 x FT ongoing Clinical Assessor - Central positions are available in our Sydney team.

Here is a typical week for a Clinical Assessor - Central:

  • You will be the initial contact for an older person and their support people, and using your patience and clinical knowledge, via telehealth (phone/video conferencing), you will perform an initial needs assessment as part of the triage process.
  • Using your decision-making skills and clinical judgement, you will determine the My Aged Care referral assessment type, eligibility status and priority/urgency in which care can be provided.
  • You will act as a point of clinical escalation for Home Support assessors and complete conversions as required.
  • Act as an authorised delegate for triage decisions, ensuring full compliance with single assessment contract rules.
  • Identify and escalate high-risk or complex cases to Senior Clinical Lead as per established work instructions.
  • Conduct risk assessments for community assessors.

Who are we looking for?

We’re looking for professional clinicians ready to make a difference—whether you're just stepping into assessment or an experienced community aged care Clinical Assessor.

To be eligible for a Clinical Assessor – Central role, you will need to:

  • Be a qualified Physio, OT, Social Worker or RN.
  • Have at least one year of experience working in aged care or with older people.
  • Demonstrate confidence when handling phone calls, building rapport quickly to complete triage calls efficiently and within expected timeframes.
  • Knowledge of the My Aged Care pathway system is desirable.
  • Hold a current unrestricted registration with AHPRA or be eligible for a relevant professional association (e.g. Registered Nurse, Physio, Occupational Therapist, or Social Worker)
  • Be an empathetic listener who has a heart for calmly and respectfully engaging with clients, families, referrers and other stakeholders.
  • Harness defined tools, technology and process which require specialised training.
  • Thrive on maintaining concise and clear documentation, with strong keyboard skills and the ability to accurately capture information in real time.
  • Be confident in using computer systems while engaging in phone conversations with ease and enthusiasm.
  • Hold a valid full unrestricted driver’s license.

We start to get to know you through your application, and you start to get to know us as we move into group video discussions (GVDs) and finish with a short series of practical tasks that let you show us what you’re made of.

Important dates:

  • We will host 1-hour group video discussions (GVD) between 16-26 June 2026.
  • If you are successful in the GVD, you will be invited to attend an online Recruitment Centre on either 1 or 2 July 2026.
  • Compliance Checks are required for suitable candidates after the RC and takes roughly 5 business days to complete.
  • Successful candidates will receive an offer, with an anticipated start date of Wednesday, 29 July 2026.
