About the role

Reporting directly to the Team Leader Client Services, you will be responsible for:

  • Supporting clients to identify suitable services and referral pathways and assisting with the development of individual strategies to promote wellbeing
  • Providing specialist and intensive assistance to help individuals impacted by dementia to understand and access aged care services and connect with other relevant supports in the community, including My Aged Care
  • Ensuring services are delivered from an enablement perspective, empowering clients to take an active role in directing their own care and services
  • Documenting all client activity and other appropriate information in line with professional and legal standards

Salary benefits: We offer all employees the opportunity to salary package up to $15,900 each year for living expenses and up to $2,650 each year for entertainment benefits.

About you

To be successful in this role, we are looking for someone with the following:

  1. Knowledge and understanding of the impacts of dementia for individuals, carers and their support networks, including the challenges that can be experienced following a diagnosis and when trying to access supports
  2. Thorough understanding of the health, community and Aged Care service sectors
  3. Proven ability to quickly build rapport with a diverse range of clients to enable effective advocacy and the achievement of individual client goals*
  4. Experience in liaising with and developing relationships with other service providers and agencies
  5. Excellent verbal, interpersonal and written communication skills, including the use of active listening and reflective questioning
  6. A formal qualification in a related field of either social work, allied health, nursing and/or significant experience working with clients in the delivery of a community-based service or within a health related field.
