The Client Services Liaison will support people living with dementia, their families and carers to enhance their quality of life through the provision of personalised information, support, referral, resourcing and innovative responsive service models. This position will promote wellbeing and build self efficacy while acting as a first point of contact to connect clients with the most appreciate service option that meets their individual needs.

Reporting directly to the Team Leader, Intake you will also be responsible for:

  • Providing a seamless experience for people living with dementia, their families and carers by providing personalised support, information, referral and other interventions
  • Engaging clients in a conversation where their needs are listened to and understood and service recommendations are well matched and appropriate
  • Delivering professional and quality services to our clients, including conducting needs assessments, making service bookings, obtaining client consent and providing responses within expected timeframes
  • Ensuring the accurate documentation and reporting of all aspects of service delivery is followed

To be successful in this role you will be a team player, have excellent interpersonal communication skills and meet the key selection criteria outlined below.

Key Selection Criteria:

  1. Experience in engaging and building rapport with clients 1:1 in a service delivery environment
  2. Ability to engage a client conversationally, to obtain relevant information to support appropriate program recommendations
  3. Knowledge and understanding of the impacts of dementia for individuals, carers and their support networks.
  4. Knowledge of service systems available to meet client needs, including My Aged Care and the Carer Gateway.
  5. Excellent verbal, interpersonal and communication skills, including the use of active listening, unconditional regard and reflective questioning.
  6. Well-developed case note and record keeping skills within the context of a client database.

Qualifications:

  • A relevant qualification in a health related field and/or significant experience working with clients in the delivery of community based services
